You can document it in the Solution screen. Click on Solution in the Fishbone drop menu in the navigation bar on top in any of the screens.
You can document as to Why the problem happened, How it could have been prevented, Solution Description, RCA Whys, Recommended Corrective Action and Lessons Learned.
